Who we are
The Forest Foot and Health Clinic will control the personal data that you provide us with.
Why we need your personal data
We use your personal data to be sure that we are treating the correct person, and are relating only your historical data to you. We are required by our statutory governing bodies to maintain a contemporaneous record of your treatment.
We will also use it to remind you of your appointments & to send you information about changes to our company or services that we provide, or for occasional marketing of new products or offers. You have the right to opt out of direct marketing at any time.
Your personal data will not be shared with a third party.
We will only collect data that is adequate, relevant and necessary for our work at The Forest Foot and Health Clinic and we will endeavour to keep your data accurate and up to date.
What we do with your personal data
Your personal data is processed fairly and lawfully, to protect your interests and only in ways that you would reasonably expect.
Your rights around the use of your personal data apply i.e.
- the right of access to a copy of the data (within 40 days of the request)
- the right to object to the processing of the data likely to cause damage or distress
- the right to refuse the use of the data for direct marketing
- the right to object to data being used in automated processes
- the right to have inaccurate data rectified, erased or destroyed.
- the right to claim compensation for damage caused by breach of the Act.
We have a data protection framework in place, to ensure that we have robust frameworks to keep your data safe.
We are required by our statutory governing bodies to retain your health records for a period of 8 years after your last visit to us. (If a child – up to their 25th year, or for mentally compromised patients – for 20 years after their last visit). After that time, it will be destroyed by shredding, and deletion from the company database.
Complaints about the collection, processing, storage or use of your personal data should be directed to the Technical Director at The Forest Foot and Health Clinic. If you are still unhappy, you can complain to the I.c.o (Information Commissioners Office).
